Grievance Procedure

Mountain View Mental Health, LLC is fully committed to conducting all activities in strict conformance with the American Psychological Association’s Ethical Principles of Psychologists. Mountain View Mental Health, LLC will comply with all legal and ethical responsibilities to be non-discriminatory in promotional activities, program content and in the treatment of program participants. The monitoring and assessment of compliance with these standards will be the responsibility of Katie Brazaitis, PhD, the Single Person Provider of  Mountain View Mental Health, LLC.

While Mountain View Mental Health, LLC goes to great lengths to assure fair treatment for all participants and attempts to anticipate problems, there will be occasional issues which come to the attention of  Mountain View Mental Health, LLC which require intervention and/or action on the part of Katie Brazaitis, PhD as the Single Person Provider of Mountain View Mental Health, LLC. This procedural description serves as a guideline for handling such grievances.

  1. When a participant files a grievance and expects action on the complaint, the following actions will be taken. All grievances will require a written note, documenting the grievance, for record keeping purposes. The note need not be signed by the grieved individual.

If the grievance concerns a speaker, the content presented by the speaker, or the style of presentation, the individual filing the grievance will be asked to put their comments in written format. This can be sent to Dr. Brazaitis on presentation evaluation forms or via the contact options below. 

Should the participant desire to speak with a third-party mediator, they will have the option of contacting Fernanda Martinez, PhD. Options for contacting Dr. Martinez are below.

Dr. Martinez will ascertain what the participant is seeking in follow-up action and offer the following:

1)    To keep a record of the complaint only with no communication to Dr. Brazaitis of the complaint or source
2)    Forward the exact complaint to Dr. Brazaitis without naming the source
3)    Provide an overview of the complaint to Dr. Brazaitis without naming the source
4)    Offer a mediated discussion between the participant and Dr. Brazaitis

And/or

5)    Provide a credit for future CE programming by Dr. Brazaitis or a partial or full refund

  1. If the grievance concerns a workshop offering, its content, level of presentation, or the facilities in which the workshop was offered, Dr. Brazaitis will address the issues to the best of her ability (seeking consultation or deferring the participant to Dr. Martinez for further mediation; see above for full anonymous options). Should the participant request action, Dr. Brazaitis will

a) attempt to move the participant to another workshop or
b) provide a credit for a subsequent year’s workshop or
c) provide a partial or full refund of the workshop fee.

Actions 2b and 2c will require a written note, documenting the grievance, for record keeping purposes. The note need not be signed by the grieved individual.

  1. If the grievance concerns Mountain View Mental Health, LLC CE program, in a specific regard, the participant may contact Dr. Brazaitis directly, or should they wish to contact a third-party mediator, Dr. Martinez.
